We are currently accepting new patients from most insurance plans. Please verify that we are in-network for your plan. We will provide you with a detailed analysis of your co-payment, deductible, coinsurance, and out of pocket expenses at your visit, as we receive them from your primary insurance carrier. This information allows patients to accurately understand any financial responsibility prior to being charged.
If your insurance requires a referral, it is your responsibility to obtain the referral prior to your visit. Your estimated portion, according to the benefits given to us by your insurance, is due at the time of service. As a courtesy, we will file your primary insurance claim for you. If you do not have health insurance or request a cosmetic procedure not covered by insurance, the payment is required at the time of service.
For your convenience, we accept cash, checks, Visa, MasterCard, and Discover. Please bring your insurance card, photo ID, and a list of medications you are currently taking to your appointment.
Additionally, if you have an HMO insurance plan, you will need an insurance referral from your Primary Care Physician (PCP).
